Primary application forms DS-11 and DS-82 must be completed online directly via the US Department of State website (see below). Upon completing the application you must print single sided with black ink on standard white paper.
The forms will download as a PDF which can be viewed directly on most internet browsers. You can also download Adobe Acrobat Reader for free if needed.
All other forms can be printed and filled out by hand. Must use black or blue pen ONLY. Original forms are required with signatures and no scratch marks or errors.

The DS-64 form is for replacing a lost or stolen passport. It serves as your formal statement explaining how the passport was lost or stolen. This form is normally provided when completing a DS-11 online through the government portal. However it can also be downloaded and filled by hand via the link below.
This form is used when one legal parent or guardian is unavailable to make a physical appearance for a child’s passport application. The parent or guardian who is unable to attend can provide consent using this form. It must be notarized and include a copy of a valid ID from the parent not present.

If you lack proof of citizenship but have lost a passport, you can opt for a File Search. Through this method the passport agency will attempt to validate your citizenship based on previous application records. This service has a separate fee of $150, which must be paid with a separate check or money order made out to the “U.S. Department of State”. The following statement must be completed and signed by hand.
