B) Proof of U.S. Citizenship |
|
You must provide the original of ONE of the following documents :
If you were born in the United States:
If you were born outside the United States, you must submit an official copy of ONE
of the following documents:
- Previous U.S. Passport
- Certificate of Naturalization Certificate
- Certificate of Citizenship
- Consular Report of Birth Abroad (Form FS-240)
- Certificate of Birth (Form DS-1350 or FS-545)
Proof of citizenship must be an original or certified copy. No photocopies or notarized copies are accepted. The original document will be returned to you with your completed passport.
If none of these documents are available, please call us at 407-440-1113 for further documentation required.

C) Proof of Identity |
|
You must present ONE of the following documents during STEP 2 (Make sure the agent makes a copy and notes on the DS-11 that you presented the ID):
- Copy of current, valid Driver's License with Photo (must be issued more than 6 months ago)
- Supplemental ID Requirements. If the above mentioned Driver's License is not available, then you need to supply an alternate Proof of Identification.

D) Two Passport Photographs |
|
You must provide two identical passport photos that meet government specifications. You can obtain passport photos at the following locations :

E) Letter of Authorization |
|
You will be emailed a link to the Priority Passport Processing Authorization Letter after you have completed your order.
|
|
F) Travel Evidence / Proof of Departure |
|
You must provide a copy of ONE of the following:
- your foreign travel plane ticket
- your printed travel itinerary showing foreign travel
- If this is a business trip, you may present a business letter from your employer. For more instructions and a sample letter, please click here.

Government Fees
» Make one check payable to "US Department of State"
- Adult Passport Book: $190
- Adult Passport Card: $30
- Adult Book and Card: $220
(this is the government processing fee).
- Be sure to include your date of birth on the check
- Starter Checks and 3rd Party Checks will NOT be accepted.
- This check will go into a sealed, government envelope in STEP 2.
» Make a second check or money order in the amount of $30 payable to the Post Office or Court House where you will visit in STEP 2.

Prior to authorization, verify that you have gathered the following travel documents:
- Government Application Form DS -11 and DS-64
- Proof of Identity
- Proof of U.S. Citizenship
- Two Identical Passport Photos
- Passport Processing Authorization Letter
- Travel Documentation
- Government fees check payable to US Department of State + $30 Fees for the Acceptance Agent.
Please make clear copies of all seven (7) documents mentioned above.
Bring all above original materials to a Passport Acceptance Facility and notify the agent you are using an expediting service to "hand carry" your application. Most facilities are open M-F, 9am to 4pm. The acceptance agent will take about ten minutes to go through the following steps :
- To verify your identity, the agent will request your proof of identity.
- The agent will ask you to sign the DS-11, and then sign and seal it themselves.
- To verify that you need an expedited passport, you will need to show them your travel documentation.
- The agent will then seal the DS-11,
DS-64, photos, proof of citizenship, travel documents, authorization letter and the
government fees check in an envelope marked "To be Opened by U.S. Passport Office Only".
- Important: Make sure the agent returns the sealed envelope to you. If the agent does not feel at ease about releasing the envelope, suggest that he/she refer to page 31 or 44 of their Passport Agent's Manual, "Hand-carrying of Executed Applications". If the agent . refuses to return the envelope, call Priority Passports at 407-440-1113 to have a representative assist you.

- Do not open this sealed envelope or the application will be invalid.
- Please write the Applicant Name, Date of Birth, and Departure Date on the outside of the sealed envelope.
- Do not seal our order form or a check to PriorityPassports.com inside the sealed envelope

1. Place the following documents in a FedEx Priority Letter.
- Sealed Government Envelope
- Payment to PriorityPassports.com
- Return Airbill or $ 20 payment to PriorityPassports.com
- Copies of seven Documents
2. Call us at 407-440-1113 and indicate you are processing an Emergency Passport from the Internet. We will instruct you on where to send your FedEx Priority Letter. Our office hours are 9am to 5pm EST Monday through Friday.
- Please have your documents when calling so we can review them with you prior to shipping.
- You will be emailed the address of where to mail your documents.
3. On the FedEx Airbill, put your Order Number in Box 2

You are sending this package to a FedEx facility for our service to receive the package to submit to the US Passport Agency by 10am. This ensures the fastest possible service; however, the Fedex facility will deny packages sent via other overnight services (UPS, Airborne, Express Mail, etc) - therefore it is imperative you send using Fedex.
